*If you don’t know or do not want to use the last four digits of the plan subscriber’s social security number, we can issue you a temporary password through the mail.


How will my e-mail address be used?

Health Plans will use the email address you provide to contact you if we have any questions about forms submitted from your account.

On occasion, Health Plans may also alert you to new web tools, health and wellness information, or request feedback. You may opt out of these types of notifications by going to "Manage My Account Settings."
